Notice of Release of Liability: This agreement contains indemnity and release provisions. In submitting this application, I (Participant/Student) am physically able to participate in any and all athletic endeavors and have not been directed by a medical authority against an activity of this type. I also understand that Karate or Martial Arts, as in all athletic activities, there is always the chance of injury.
In consideration of the risk of injury that exists while participating in Karate/Martial Arts, Participant, which includes parents or guardians, knowingly and voluntarily enter into this Waiver and Release of Liability and hereby waive any and all rights, claims or causes of action arising out of my participation while training, competing, officiating or otherwise participating in said activity and release and forever discharge Mandeville Karate, Scott Shook and their affiliates, assignees, managers, attorneys, staff and volunteers, from physical or psychological injury Participant may suffer as a direct result of participation in the aforementioned Karate/Martial Arts.
I agree that this Release shall be governed for all purposes by Louisiana law, without regard to any conflict of law principles. This Release supersedes any and all previous oral or written promises or other agreements.
In the event that any damage to equipment or facilities occurs as a result of Participant or Participant’s family’s or agent’s willful actions, neglect or recklessness, Participant/Guardian acknowledges and agrees to be held liable for any and all costs associated with any such actions of neglect or recklessness.
